* Note: All EMS Course candidates must be able to sign an NYSDOH application for Emergency Medical Services Certification (DOH-65), which states the following:
I affirm that in accordance with the requirements of 10 NYCRR 800, I have NOT been convicted of any misdemeanors or felonies. I understand that if I have a conviction it will be individually reviewed and that any such conviction may not be an automatic bar to certification. The Department of Health will determine if the conviction is applicable under the provisions of Part 800.
Individuals who are not able to sign the above affirmation, may enroll in a NYS sponsored EMS course, but will not be allowed to take the NYS Written Certification exam until such time as the Department of Health determines the impact of the individuals criminal record on that individuals suitability to practice. As such, the course sponsor (Mobile Life Support Services) can not voucher the Health Department for the course tuition costs associated with the individual until the individual has been cleared by the Department and then takes (and passes) the Written certification exam.
Experience with this process has shown us that in such cases we must require that such candidates pay the tuition for the course or have their sponsoring EMS agency pay the tuition on their behalf. If the latter is done, the sponsoring EMS agency can then voucher the Health Department for tuition reimbursement when the individual has been cleared by the Department and has passed the certification exam.
